Address issues and Peacock Premium

There are multiple addresses associated with my account since I moved services to a new address in October.  Need to remove all but the current one.  

Also, I'm supposed to have Peacock Premium as part of my plan but I don't.  I can't activate it, and I'm thinking perhaps it's due to the multiple address issue.

I need the addresses fixed, and my Peacock Premium (FREE) restored to my account.
