Comcast underground cable damage claim

I am writing to file a formal property damage claim against Comcast/Xfinity regarding damage to the concrete driveway at my property located at

The damage — consisting of cracked and lifted concrete sections — is located adjacent to a Comcast underground cable access box on my property. The displacement of the concrete is consistent with subsurface infrastructure movement beneath the driveway surface and has created an uneven joint with surrounding concrete surfaces.

This condition constitutes a trip hazard and presents a liability risk to anyone accessing the property. I have attempted to contact Comcast previously to resolve this matter without success.

I would like to respectfully request the following:

1. A site inspection by a Comcast representative or authorized contractor
2. Written confirmation that this claim has been received and assigned a claim number
3. Repair or reimbursement for repair of the damaged concrete caused by Comcast's underground infrastructure
