Tokenization request is declined. Reason Code: 481 Reason Message: The order has been rejected by Decision Manager

I was trying to add a new mobile line. However, this message pops up on the last step when I hit submit:

Tokenization request is declined. Reason Code: 481 Reason Message: The order has been rejected by Decision Manager

it still shows up even after I switched to another card with another bank. Please help me fix this issue
