$200 credit

Hi, last month I signed up for xfinity mobile and part of the reason was the $200 credit (per line?) which was supposed to incentivize me to break from my prior carrier. Now it's September and the offer is no longer advertised. When I check my bill, it's unclear I am getting the credit. 

1. Was the credit per line or just for one line (regardless of multiple lines in my account)?

2. How do I get the credit?
