Transfer mobile service to new address

Hello, I recently moved to a new address. I ordered a new wifi modem for the new address and have installed it. However, my account now seems to be separated into two, and I can no longer see or access my mobile plan on the Xfinity app, as it only shows my wifi information. How can I remove the old account and have my mobile plan information transfer to my new address?
