Completing live agent evaluation that did not display at the end of our conversation

I am looking for a way to provide both an excellent evaluation of a recent live agent chat for my account at ending with  ...xxxx xxx [Edited: "Personal Information"] at address [Edited: "Personal Information"]  The usual pop up survey did not start and I want to make sure that is excellent work gets credited.  [Edited: "Personal Information"]
